Your Rights As A Patient

All of us at Our Lady of Fatima Hospital want your stay to be as pleasant as possible. If during your stay, or after you have been discharged, you or your family have any questions or concerns you can contact the nurse in charge on your floor or contact our Patient Relations Office by dialing extension 3888 from your bedside phone or 456-3888 from outside the hospital. This number is staffed Monday through Friday, from 8:30 a.m. to 4:30 p.m. On off hours, the nursing supervisor on your floor is available to handle urgent issues.

Inpatients who are discharged home may receive a patient satisfaction survey in the mail. We encourage you to complete this and return it in the envelope provided.
